Ordering by mail?

I'm one of those odd ducks who don't use credit cards.  So...I tried to click on the "order by mail" portion in hopes to get the requisite info, but the link only sent me right back to the same page.  Tiring of this circuitous routine, I thought I'd ask here how to do this!
